CISD is partnering with Tom Thumb Pharmacy to provide COVID-19 vaccine clinics for children ages 5 and up and for district community members who would like the vaccination or need a booster.
The clinics will be held on Wednesday, Nov. 17 and Wednesday, Dec. 8 in the gym of Wilson Elementary School from 3:30 to 7 p.m. Wilson Elementary is located at 200 S. Coppell Road in Coppell. Tom Thumb will provide the Pediatric Pfizer vaccine for 5 to 11-year-olds, the Adult Pfizer vaccine for those ages 12 and up and the Moderna vaccine for those over the age of 18. Please fill out the English Consent Form or Spanish Consent Form after making the appointment and bring the form with you. Forms also will be available on site. Please indicate your relationship to the patient on the signature line. A consent form is needed for each clinic. A parent or guardian must accompany a child under the age of 18 at the clinic.
